UNICARE HOSPICE in Yuma provides compassionate care that focuses on all aspects of an individual’s life and well-being: physical, social, emotional, and spiritual. For those in our care, Unicare Hospice’s interdisciplinary team consists of the patient’s physician, a hospice doctor, a case manager, registered nurses and licensed vocational nurses, a counselor, a dietitian, therapist, social workers, a chaplain, and trained volunteers.

You become eligible for hospice services if your serious illness is considered terminal and your doctor feels that your prognosis is likely to be six months or less. You may revoke from Hospice at any time or Hospice may discharge you if your health sufficiently improves.

Hospice operates on a daily per diem basis within four levels of care:

Respite Care:  While on routine daily care, Hospice can arrange for you to stay for up to 5 days in a Medicare-certified nursing facility if your family caregivers should need a “breather” from the intensity of caregiving.

Crisis Care: When your comfort cannot be control under intermittent skilled visits, a nurse and other team members may be required to assess, monitor, and treat symptoms up to 24-hours/day until Palliative comfort care is achieved.

Inpatient Care.  Occasionally, there may be times when you’ll need to be in a Hospital or acute Medicare-certified facility. our entire hospice team, including your private physician, determined this necessity and remain directly involved with your care and with your family.
